Kecelakaan kerja bisa berakibat pada kematian. Dampak secara luas akan menyebabkan kerugian pada perusahaan karena kehilangan tenaga kerja. Dampak dari itu adalah menurunnya produktivitas kerja. Salah satu faktor penyebab kecelakaan kerja adalah perilaku tidak aman yang dikarenakan rendahnya pengetahuan pekerja akan keselamatan dan kesehatan kerja (K3). Perilaku tidak aman dapat diminimalisir dengan adanya implementasi sistem manajemen keselamatan dan kesehatan kerja (SMK3) yang baik dan penilaian ergonomi terkait lingkungan kerja fisik dan postur kerja dari pekerja. P.T. IGLAS (Persero) merupakan perusahaan yang memproduksi botol-botol kaca. Berdasarkan data tempat terjadinya kecelakaan kerja, bagian forming menempati posisi tertinggi. Tujuan dari penelitian ini adalah mengevaluasi implementasi SMK3, dan meningkatkan implementasi SMK3 melalui penilaian ergonomi (postur kerja dan lingkungan kerja fisik) dan identifikasi perilaku tidak aman. Hasil pengukuran implementasi SMK3, P.T.IGLAS berada pada level 3 yaitu “hati-hati” dengan indikasi warna kuning. Pada penilaian lingkungan kerja fisik didapatkan faktor suhu, cahaya, kelembaban, dan kebisingan yang tidak sesuai dengan nilai ambang batas. Pada postur kerja dengan menggunakan metode Rula, didapatkan aktivitas yang dilakukan oleh operator memiliki level resiko sedang sampai tinggi (5-7). Pada identifikasi perilaku tidak aman, hanya faktor pengetahuan yang berpengaruh secara signifikan, persepsi, sikap, motivasi, dan pendidikan tidak berpengaruh secara signifikan, dengan nilai variabel Y (perilaku tidak aman) adalah 12,008 dan R square adalah 96,8%. Berdasarkan hasil analisa, untuk meningkatkan implementasi SMK3 di P.T.IGLAS terdapat beberapa usulan perbaikan antara lain: wajib dibentuknya penanggung jawab lapangan atau penanggung jawab kesehatan dan keselamatan kerja (PJ K3) dari setiap area, pengadaan ventilasi turbin untuk menciptakan sirkulasi udara yang baik, khususnya di area forming, meredesain meja yang ergonomis dan penggunaan alat pelindung diri (APD) yang sesuai dengan area kerja.

The work accidents can may result in death. The impact will cause harm to the company because the company can experience loss of employment. The impact of this could be the decline in labor productivity. One of the causes of accidents is unsafe behavior. It is because the knowledge of K3 (kesehatan dan keselamatan kerja) from worker is low. The unsafe behavior can be minimized with good implementation of SMK3 (sistem manajemen kesehatan dan keselamatan kerja) and ergonomic assessment (e.g.: the physical work environment and work postures of workers). P.T. IGLAS (Persero) is a company that produces glass bottles. Based on data from workplace accidents, the forming departement has level of the highest level of accidents at work. The purpose of this study is the implementation evaluation of SMK3, and the improved implementation of SMK3 and ergonomics assessment (work postures and physical work environment) as well as the identification of unsafe behavior. The measurement result shows that the SMK3 implementation in P.T.IGLAS was at the level of 3 which shows the ‘’careful’’ sign represented by the yellow color indication. The assessment of the physical work environment comprise the factors of temperature, light, humidity, and noise that do not comply with the threshold value. In the measurement of working postures with Rula method, it is known that the activities by operators have moderate to high level of risk with the value of 5-7. In the identification of unsafe behavior, just the factors of knowledge significantly with the Y variable, perception, attitude, motivation, and education did not significantly influence the Y variable (unsafe behavior) the computed value of Y is 12,008 and the R-square is 96,8%. Based on the analysis, the results show that to improve the implementation SMK3 in P.T.IGLAS, there are many proposed improvements, which include to assigning person in charge in each area (PJK3) (penanggung jawab K3), procurement turbine ventilator to create good air circulation, (especially in the forming area), ergonomic redesign of the table, and use of good APD (alat pelindung diri) to the work area.
