Kebutuhan dan persaingan di industri pakan ternak ayam yang terus meningkat, membuat perusahaan-perusahaan pakan ternak ayam di Indonesia akan meningkatkan kualitas produk dan prosesnya untuk dapat meningkatkan pangsa pasar penjualan produk di Indonesia. PT. Japfa Comfeed Indonesia. Tbk Unit Margomulyo merupakan salah satu perusahaan pakan ternak ayam di Indonesia. Pada area entrance prosentase tolakan jagung sebesar 20%, dibagian proses produksi ditemukan terjadi re-produksi pakan sebesar 101.600 kg dan pada area gudang tingkat kutu untuk bahan baku dan pakan jadi masih relatife tinggi. Dari temuan tersebut maka dilakukan penelitian untuk mengurangi tingkat ketidaksesuaian di area entrance, proses dan gudang menggunakan metode six sigma dan FMEA. Berdasarkan hasil penelitian dapat disimpulkan bahwa terdapat 4 area yang terkait dengan produksi pakan ternak ayam di PT. Japfa Comfeed Indonesia Unit Margomulyo, yaitu area Entrance, In Process, Stock Control Raw Material dan Stock Control Finish Product. Area In Process memiliki capaian nilai sigma tertinggi (5,56) dengan nilai DPMO terkecil 104,673. Sedangkan bagian Stock Control Raw Material dan Entrance memiliki capaian tingkat sigma terendah masing-masing (2,93) dan (3,31) dengan nilai DPMO masing-masing mencapai 25117,4 dan 13468,92. Penyebab-penyebab ketidaksesuaian yang terjadi di area Stock Control Raw Material sebagian besar disebabkan oleh faktor material, metode dan lingkungan. Sedangkan untuk area Entrance didominasi oleh faktor personnel dan metode. Berdasarkan fokus permasalahan tersebut maka tindakan perbaikan untuk ketidaksesuaian hanya dilakukan pada area Stock Control dan Entrance.
